Providence Hospice & Palliative Care

Providence Willamette Falls Hospice makes it possible for our patients to spend their final time at home or in a homelike setting (nursing homes, assisted living, adult foster homes etc.), in the presence of family and friends. We are committed to providing comfort and quality of life to both patient and family, enabling them to live as fully and comfortably as possible through the end of life process.

Our staff is trained in end of life care, specifically pain and symptom management, emotional and spiritual support, and bereavement support following the death of a loved one. To contact Hospice for questions or to set up a meeting, please call 503-655-7581.

The Providence Willamette Falls Hospice Team includes:

  • Patient’s Primary Physician
  • Hospice Medical Director
  • Registered Nurses (available seven days a week)
  • Medical Social Workers
  • Spiritual Counselors
  • Home Health Aides
  • Physical, Occupational and Speech Therapists
  • Dietician Services as needed
  • Trained Volunteers
  • Bereavement Counselor**
  • Hospice also provides medications, durable medical equipment, and supplies related to the terminal illness.

    ADMISSION and ELIGIBILITY

    A person is eligible for hospice if they have a physician who certifies that life expectancy is six months or less. Anyone may refer a person to hospice. We service those clients living within a 30-mile radius of Oregon City.

    COVERAGE

    Hospice is a benefit of Medicare and Medicaid (the Oregon Health Care Plan). Many private insurance companies and HMOs have a hospice benefit. Willamette Falls Hospice accepts and serves all regardless of their ability to pay.
